Office Policies
Clear policies help us focus on what matters most: your care. Here is what to expect from us and what we ask of you.

Office Hours
Monday through Thursday: 9:00 a.m. to 5:00 p.m.
Friday: 9:00 a.m. to 4:00 p.m.
Office staff are available during these hours for scheduling, questions, and general support.
Appointments and Cancellations
Cancellation policy.
Please provide at least 24 hours' notice if you need to cancel or reschedule. This allows us to offer your appointment time to another patient who may be waiting for care. A fee may apply for missed appointments or cancellations made with less than 24 hours' notice. Specific fee details are provided during the intake process and in your signed financial agreement.
New patient deposit.
A deposit is required to hold new patient appointment times. The deposit is applied toward the cost of your visit. It is not an additional fee. If you no-show or cancel with insufficient notice, the deposit may be forfeited.
Late arrivals.
If you arrive more than 15 minutes late, your appointment may need to be rescheduled. To ensure every patient receives the full time they deserve, we may need to reschedule if a late arrival does not leave enough time for a thorough visit. A cancellation fee may apply if rescheduling is necessary and notice was insufficient.
Repeated missed appointments.
We are committed to working with every patient. However, consistent missed appointments affect your continuity of care and our ability to serve other patients. Repeated no-shows may lead to discharge from the practice.
Insurance and Payment
Payments, including copays, deductibles, and self-pay fees, are due at the time of service. We accept cash, credit card, and debit card.
Please keep your insurance information current and notify us of any changes at least 24 hours before your appointment.
For patients with out-of-network or non-accepted insurance, we provide a superbill for you to submit to your insurance for potential reimbursement. We cannot guarantee reimbursement.

Prescriptions and Refills
Prescription refills are managed during your scheduled visits. Your provider reviews your treatment, makes any necessary adjustments, and sends prescriptions as part of that appointment.
Refills for controlled substances require a visit. This is standard clinical practice and is in place for your safety.
If you are concerned about running out of medication before your next appointment, contact the office as soon as possible so we can help.
Communication Between Visits
For non-urgent questions or concerns between visits, contact the office by phone during business hours.
Messages are handled through office staff. During business hours, expect a response the same day or next business day. After hours, messages are returned within 24 to 48 hours. For weekends and holidays, messages are returned when staff returns, typically by the end of the first business day.
Providers do not typically call patients directly. Communication is routed through the office to ensure accurate documentation and timely follow-up.
After-Hours and Emergencies
For non-urgent after-hours questions, leave a voicemail. The office will return your call when staff returns on the next business day.
We are not a crisis service. If you are experiencing a psychiatric emergency, please call 911, go to your nearest emergency room, or call the 988 Suicide and Crisis Lifeline.

Mental Capacity and Disability Evaluations
A fee of $250 applies for completion of mental capacity or disability evaluation forms. This requires a separate assessment visit dedicated to the evaluation. It cannot be completed during a regular office visit.
The assessment visit is not covered by insurance and is the patient's responsibility. Completed paperwork is available approximately one week after the assessment.
Court Appearance Requests
If a court appearance by a provider is requested, a fee of $350 per hour applies. Payment is required in advance.
Court appearances require significant preparation and time away from patient care.
Medical Records
Patients may request copies of their medical records. Requests are processed through the office by phone or in writing. Fees may apply.
Our Commitment to a Respectful Environment
We believe the best outcomes come from a collaborative relationship built on trust and mutual respect. We are committed to working with every patient, and we also have a responsibility to maintain a safe and respectful environment for everyone.
Situations that may lead to discharge from the practice include:
- Verbal abuse or threatening behavior toward staff or other patients
- Noncompliance with treatment recommendations
- Noncompliance with office policies
- Nonpayment
- Improper use of prescribed medications
- Disruptive behavior

Patients who need assistance completing online forms or who lack access to technology can contact the office to arrange in-person support. Call us to schedule a time. Our staff will be happy to help.
