Completing Your Paperwork Online:

Before your visit, you'll receive a link to our secure online portal where you can complete your intake and assessment forms. This process is essential for gathering comprehensive information about your health history, current concerns, and how we can best assist you. Completing these forms in advance helps us prepare for your appointment and ensure we can focus on discussing your needs and treatment options.

Steps to Complete Your Online Paperwork:

  1. Check Your Email: Look for an email from our clinic with a link to the online forms.
  2. Secure Portal Access: Click the link to access our secure portal. You'll need to create an account or log in with a valid email address.
  3. Fill Out the Forms: Complete the intake and assessment forms at your convenience before your appointment.
  4. Finalize and Schedule your Appointment: Once you've submitted everything, a staff member will contact you to finalize and schedule your first appointment.

Need Help with Technology or Don't Have Access?

We understand that not everyone is comfortable with technology or may not have access to a smartphone or computer. If you're finding it difficult to use technology to complete your paperwork or lack the necessary equipment, we're here to help:

  • In-Office Assistance: Come to our office where one of our friendly staff members can assist you with completing the paperwork on our computer. This service ensures that everyone, regardless of technological proficiency or equipment availability, has equal access to our services.
  • Schedule a Time for Help: To ensure we can provide you with the assistance you need, please call us in advance to schedule a time to come in. This helps us maintain social distancing and ensure a staff member is available to assist you.

What to Expect During Your First Visit:

Once your paperwork is complete, your first visit will focus on understanding your concerns and discussing the next steps. This visit is an opportunity for you to share your experiences, concerns, and goals for treatment. Based on the information provided in your online forms and our discussion, we'll work together to outline a treatment plan tailored to your needs.

Preparing for Your Visit:

  • Bring Identification and Insurance Information: Please bring a photo ID and any relevant insurance cards.
  • Arrive Early: If you haven't completed your paperwork online, or if you're scheduled for in-office assistance, please arrive at least an hour before your appointment time.
  • List of Medications: If you're currently taking any medications, please bring a list of these, including dosages. If you are completing paperwork online, please make sure fill out current and past medication sections completely.
