Our office staff are here to assist you during these hours. Should you need to contact us after hours, please leave a message, and we will get back to you as soon as possible. For emergencies, proceed to the nearest emergency room or call 911.
For non-urgent issues, leave us a message, and we'll respond during office hours. In urgent or life-threatening situations, always call 911. For immediate support in non-medical emergencies, contact our crisis hotline or other useful numbers provided.
Life happens, and we understand that. Please cancel at least 24 hours in advance to avoid a no-show fee and allow us to reschedule you at a more convenient time. Remember, keeping track of your appointments is crucial, and repeated no-shows may lead to discharge from our services.
Ensure we have a copy of your insurance card. Notify us of any changes 24 hours in advance. For self-pay patients or those with non-accepted insurance, we provide receipts for potential insurance reimbursement. However, we cannot guarantee reimbursement. Payments are due at the service time, and we accept cash, credit, or debit cards.
A fee of $250 applies for completing these forms. An assessment visit, separate from regular office visits, is required and is not insurance reimbursable. Completed forms are available one week after the assessment.
Our providers focus on delivering quality mental health services. Court appearances by our providers incur a fee of $350 per hour, payable in advance.
We value a trust-based provider-patient relationship. Situations that may lead to termination include treatment noncompliance, missed follow-up appointments, verbal abuse, noncompliance with office policies, nonpayment, improper use of medication, and disruptive behavior.
